Root Cause Analysis

The Root Cause Analysis (RCA) is an outstanding method for recording errors and digging into the root of a problem using a systematic form of questions and answers. The RCA involves the 5 whys of critical thinking. Although difficult to truly complete, the process of completing an RCA forces you to use critical thinking and provides important information that can be analyzed by the whole team.
